January 5, 2026FEATURED MEDICAL EDUCATION TOPIC: Unanticipated Exits from the Hospital
What is the problem?
Hospitals increasingly face challenges with unanticipated patient exits, including patients who leave Against Medical Advice (AMA), leave before an evaluation is complete, leave without being seen (LWBS), or elope from the facility. These events are more than operational disruptions; they are patient-safety issues with significant clinical and legal consequences. Patients who leave the hospital before formal discharge are at higher risk of adverse events and are more likely to be involved in malpractice allegations. Yet many clinicians rely on outdated assumptions or an incomplete understanding of the requirements for proper management. It is important to equip physicians with the knowledge and skills to manage these situations safely and appropriately.
Learning Objectives
At the conclusion of this activity, the learners will be able to:
- Develop an evidence-based approach to patients who leave prior to discharge.
- Identify high-risk myths surrounding AMA departures and correct them.
- Differentiate between LWBS patients and those who leave before evaluation is complete.
- Understand the risks and reporting requirements for true elopements
- Improve care for patients with cognitive impairment, whose symptoms may contribute to unsafe departures
